URL & test conditions

Record the URL, device or template, timing and reproduction steps. Do not turn a problem on one page into a site-wide claim without checking.

Impact & priority

Explain the impact on visitors or search-engine access. Separate broken functionality from improvements that can wait.

Fix & dependencies

State the proposed fix, required access and related tasks. A URL change needs redirect and internal-link checks, not just a renamed page.

Choose the issue group, approver and a suitable backup or recovery method. An audit does not by itself include permission to modify the live site.

Make changes in a suitable environment

Use a test environment for substantial changes where possible. Record modified files, settings or content for verification and handover.

Check related functionality

Besides the original issue, check affected forms, navigation, mobile layouts and pages sharing the same template.
